  1. Gather Information: You will need the Company Name, USDOT Number (if visible), and the License Plate of the truck.
  2. Visit the NCCDB: Go to the National Consumer Complaint Database.
  3. Select Category: Choose "Safety Violation" from the list of options.
  4. Submit Detail: Be as specific as possible about the location, time, and nature of the incident.
